Insurance & Forms

Welcome to the Texas Urogynecology & Laser Surgery Center. Here are some steps to help make your visit go smoothly.

Every new patient is required to read the “Notice of Privacy Practice” brochure. Each patient must fill out the HIPAA Acknowledgement form, Patient Information Sheet, and Medical Questionnaire.

    Setting Your Appointment

    Call 915-533-5600 during normal business hours. Please advise our staff if you are being referred or have had a change in your insurance policy. Please have the following information available: Full name, date of birth, insurance information, and preferred pharmacy.

    Office Hours: Monday – Friday – 9 am to 5 pm

    Payment / Insurance Information

    Please have your insurance card available while making your appointment.

    Please advise us of any of the following:

    • Subsidiary Medicare program
    • Secondary Insurance policy
    • Out-of-State policy.

    We accept:

    • Medicare
    • Tricare
    • Most Forms of Commercial Insurance.

    Contact our office for details. Our office staff will contact your insurance provider to verify all benefits. However, it is the patient’s responsibility to make sure that their policy is active and to be aware of any limitations of coverage.

    Bring Any Medications & Previous Records

    It is very important for all new patients, referred patients, or patients that have seen Dr. Farnam at another practice location to obtain all pertinent records prior to your appointment. You are welcome to have your records faxed to our office at 915-533-5600. However, to ensure the availability of your information during your appointment and to prevent a delay in your visit, we recommend that you obtain a printed copy of your records and bring it with you to your appointment. Please also bring any medications you are taking.

    The Texas Gynecology and Laser Surgery Center is conveniently located in the heart of the El Paso medical district in the Oregon Medical Building. Patients that are new to the center should arrive 30 minutes prior to their scheduled appointment to complete the medical forms and allow for verification of insurance benefits.
